Services funded by Support At Home Program — we help you get the most from your package and work closely with your care partner for smooth care.
The Support at Home program is bringing a simpler, more flexible way to access in-home aged care. This system has replaced Home Care Packages and Short-Term Restorative Care, combining them into one easy-to-navigate program designed to better match your care needs and goals.

Funding Changes Made Simple
Your funding will continue to be determined through an aged care assessment, but there’s now a clearer, more tailored structure.
Under the current Home Care Package system, there are four levels of funding. The Support at Home program introduces eight classifications, allowing for more personalised care and support, with your annual budget distributed quarterly for easier management.
Azure Specialists Home Care Services will continue to provide the same compassionate care and professional support, while helping you transition smoothly to the new program.

Support at Home services
Support at Home services fall into three categories:
- Clinical Care – Professional support from registered nurses and allied health specialists, including physiotherapy, occupational therapy, speech pathology, and podiatry to keep you healthy and confident at home.
- Independence Support – Assistance with showering, dressing, medication reminders and other personal care to help you remain independent.
- Everyday Living – Assistance with cleaning, laundry, grocery shopping, meal preparation, and gardening to keep your home comfortable.
Your care coordinator will work with you to review goals, budgets and services — ensuring minimal disruption and clear next steps.
Funding levels are determined through the aged care assessment process. We help you understand and manage your budget across the year.
